Dear John,

The good news is that agoraphobia and panic disorder are curable, so do find a good therapist.

The other good news is that illness dispenses you from your Mass obligation. Also, a priest can come to you for Reconciliation and someone can bring you the Eucharist, however, you say you find it nearly impossible, not impossible, to function outside your home so I am wondering under what circumstances you are able to function.

You might transfer that set of circumstances to the Mass attendance situation and have some success.
